I carry major medical only with a $2K deductible. Since I am self employed the PPO and HMO's are hundreds of dollars each month. When we go I simply say I am paying cash, no insurance. They cut the office fee from $75 to $50. So, we would have to go more than 4 times a month to show any gain.
You can then open an HSA (health savings account)for the $2k. this is a pretax deduction and as long as it is used it lowers your taxable income. They issue you a medical debit card. The doctor off debits out of that account.
Then, major medical covers 85% of pmajor procedures.
